Dianabol: Side Effects and Organ System Health Risks
Dianabol contains methandrostenolone, also called methandienone or metandienone, a 17 alpha methylated oral anabolic androgenic steroid with documented effects on the liver, reproductive hormones, blood pressure, fluid balance and androgen sensitive tissues. Human methandienone research has recorded increased systolic blood pressure, water associated body weight gain, marked suppression of natural testosterone, reduced gonadotropin output, abnormal liver tests and treatment limiting adverse effects.
Liver injury has unusually strong compound specific evidence. Human methandienone studies recorded changes in hepatic excretory function and liver enzymes. Case literature includes severe cholestatic illness, fatal liver failure and a ruptured benign liver cell adenoma after prolonged exposure. Methandrostenolone belongs to the carbon 17 alpha alkylated androgen group that LiverTox associates most strongly with cholestasis, peliosis hepatis and hepatic tumors.
Not every effect is visible. Blood pressure, liver tests, endogenous testosterone, gonadotropins, cholesterol, triglycerides and sperm production can change before a person feels seriously unwell. Feeling normal does not establish normal organ function.

What are the main side effects of Dianabol?
Methandrostenolone has direct human research dating from the period when Dianabol was still used as a pharmaceutical medicine. That makes it possible to separate several compound specific findings from effects inferred from anabolic steroids as a wider drug family.
Controlled research in male weightlifters found significantly increased systolic blood pressure, body weight gain that appeared partly related to water retention and numerous adverse effects, with several participants leaving the study because of them.[1]
Separate endocrine research recorded marked suppression of blood and urinary testosterone and some suppression of pituitary gonadotropin output during methandrostenolone exposure.[2]
Longer term observations in bodybuilders using methandrostenolone found low testosterone, reduced luteinizing hormone, abnormal liver function tests in half of the small group and metabolic abnormalities in one participant.[3]

How strong is the evidence for common Dianabol side effect claims?
Not every claim repeated about Dianabol has the same evidential footing. Some effects were recorded in controlled methandienone experiments. Others are supported mainly by clinical cases or by research on carbon 17 alpha alkylated anabolic steroids as a family.
| Claim | Best evidence | Evidence reading |
|---|---|---|

| Every adverse effect reverses after stopping | Contradicted by some androgenic and hepatic outcomes | False |
Why is Dianabol associated with liver injury?
Methandrostenolone is a carbon 17 alpha alkylated androgen. The methyl substitution at carbon 17 helps the active steroid survive oral administration, but compounds in this structural family have the strongest association with androgen related cholestatic liver injury.
LiverTox includes methandrostenolone among the carbon 17 alpha alkylated anabolic steroids and describes four broad hepatic patterns associated with this drug family: temporary enzyme abnormalities, acute cholestatic injury, peliosis hepatis and hepatic tumors.[4]
Compound specific evidence exists as well. A historical study of hepatic function during methandienone therapy followed 30 patients and found abnormal sulfobromophthalein retention in a majority of participants, while AST elevations occurred in a smaller group. Some abnormalities appeared without symptoms.[5]
| Finding | What may happen | Possible signs | May be silent? |
|---|---|---|---|
| Abnormal liver function tests | AST, ALT or hepatic excretory measurements can change | Often none | Yes |
| Cholestasis | Bile transport becomes impaired | Jaundice, itching, dark urine and pale stool | Early disease can be subtle |
| Peliosis hepatis | Blood filled spaces form within liver tissue | Often none until complicated | Yes |
| Hepatic adenoma | A benign liver cell tumor develops | Often none, or abdominal pain if large or ruptured | Yes |
| Severe liver failure | Hepatic function deteriorates substantially | Jaundice, confusion, bleeding and severe illness | No once advanced |
Read anabolic steroids and liver damage for the wider hepatic evidence.
What does cholestatic liver injury from Dianabol look like?
Cholestasis means bile formation or bile flow is impaired. Carbon 17 alpha alkylated androgens are well known for causing a pattern in which jaundice and itching can become pronounced even when aminotransferase elevations are not as dramatic as those seen in some forms of acute hepatitis.
A historical report described fatal liver failure after methandrostenolone treatment in an older woman being treated for osteoporosis. Jaundice developed after exposure, followed by progressive deterioration, coagulopathy and death. Autopsy findings included marked cholestasis.[6]
| Feature | Possible presentation |
|---|---|
| Bilirubin | Can rise substantially as jaundice develops |
| ALT and AST | May rise, but the increase does not always reflect the severity of jaundice |
| Urine | Can become unusually dark |
| Stool | Can become pale when bile delivery to the intestine falls |
| Skin | Generalized itching may become severe |
| Eyes and skin | Yellow discoloration may appear |
| Severe disease | Confusion, bleeding, weakness or clinical deterioration may occur |
A normal appearance or absence of pain does not exclude early liver dysfunction.
Is Dianabol associated with liver tumors or peliosis hepatis?
Long term exposure to androgenic anabolic steroids has been associated with hepatic adenoma, hepatocellular tumors and peliosis hepatis. Methandienone has direct case evidence for hepatic adenoma.
A 1977 report described a 19 year old man who had received methandienone for three years for paroxysmal nocturnal hemoglobinuria. He was admitted with bleeding into the abdominal cavity after rupture of a hepatic tumor. Histology identified a benign liver cell adenoma.[7]
The report does not establish the incidence of adenoma in every methandrostenolone user. It establishes that a serious hepatic tumor association has been documented in a person exposed to the compound for an extended period.
| Outcome | Nature of evidence | Why it matters |
|---|---|---|
| Hepatic adenoma | Direct methandienone case evidence | Adenomas can enlarge or rupture and bleed |
| Peliosis hepatis | Strong androgen class evidence | Blood filled cavities can cause internal bleeding |
| Hepatocellular tumors | Long term anabolic androgen evidence | Both benign and malignant tumors have been reported |
Sudden severe upper abdominal pain, collapse or signs of internal bleeding need emergency assessment.
Can Dianabol raise blood pressure?
Yes. Methandienone has direct controlled human evidence for increased systolic blood pressure.
A crossover trial involving experienced male weightlifters compared methandienone with placebo. Systolic blood pressure increased significantly during methandienone treatment. Body weight also increased, with investigators judging that much of the change appeared related to water retention.[1]
Blood pressure elevation may produce no symptoms. Headache, flushing or dizziness can occur, but symptom absence does not establish a normal measurement.
| Factor | Possible contribution |
|---|---|
| Fluid retention | Can increase circulating volume and body weight |
| Systolic blood pressure | Directly increased in controlled athlete research |
| Blood lipids | Adverse lipid patterns can add long term vascular risk |
| Other stimulants or anabolic steroids | Can add separate blood pressure and cardiovascular effects |
| Existing hypertension | Leaves less margin for a further increase |
Read anabolic steroids and cardiovascular function for the broader cardiovascular evidence.
Does Dianabol cause water retention?
Water associated weight gain is supported by controlled human evidence. In the 1975 weightlifter trial, body weight increased during methandienone exposure, and investigators considered water retention an important contributor.[1]
A separate double blind crossover experiment found increased body weight and a large increase in body potassium during methandienone exposure. The investigators noted that some apparent lean tissue increase could alternatively have reflected increased intracellular fluid.[8]
| Contributor | Possible effect on body weight |
|---|---|
| Intracellular water | Can increase apparent lean body mass |
| Extracellular fluid | Can contribute to swelling and scale weight |
| Glycogen associated water | Can increase muscle water content |
| New tissue | May contribute to body composition change over time |
| Fat mass | Can change independently through food intake and energy balance |
A rapid increase on the scale cannot be interpreted as an equivalent increase in newly formed muscle tissue.
Can Dianabol affect cholesterol or triglycerides?
Yes, though direct methandrostenolone studies do not provide one uniform lipid response that applies to everyone.
Blood cholesterol was monitored in controlled athlete research because lipid effects were already considered relevant. Longer term observations of six bodybuilders using methandrostenolone found one participant with high serum cholesterol and triglycerides together with mild diabetes, while abnormal liver tests were seen in several others.[3]
The broader evidence for oral anabolic androgenic steroids supports concern about reduced high density lipoprotein cholesterol and increased atherogenic lipid exposure. Carbon 17 alpha alkylated oral steroids can produce marked lipid changes in some users.
| Claim | Evidence reading |
|---|---|
| Methandrostenolone can affect lipid metabolism | Supported by human and anabolic steroid family evidence |
| Every person develops the same cholesterol pattern | Not supported |
| A person can feel a fall in high density lipoprotein | No. Lipid changes are generally silent. |
| Normal fitness proves a favorable lipid profile | No |
| Other oral steroids can add to the same lipid burden | Biologically plausible and supported by anabolic steroid evidence |
What cardiovascular risks are associated with Dianabol?
Methandienone specific evidence documents increased systolic blood pressure and fluid associated weight gain. Wider anabolic steroid research adds concern about dyslipidemia, vascular dysfunction, cardiac remodeling, impaired ventricular function, thrombosis related factors and atherosclerotic disease.
It is useful to keep those levels of evidence separate. A finding established across long term anabolic steroid users should not automatically be described as if a methandrostenolone only randomized trial proved the same outcome.
| Outcome | Methandrostenolone specific evidence | Wider anabolic steroid evidence |
|---|---|---|
| Higher systolic blood pressure | Direct controlled evidence | Also supported across the drug family |
| Fluid retention | Direct controlled evidence | Common androgen related concern |
| Adverse blood lipids | Limited direct observations | Strong family level evidence |
| Cardiomyopathy | No strong methandrostenolone only trial evidence | Associated with long term anabolic steroid exposure |
| Atherosclerotic disease | Not quantified for methandrostenolone alone | Supported by longer term anabolic steroid research |
Chest pressure, sudden severe breathlessness, collapse, new neurological weakness or trouble speaking needs emergency care.
Does Dianabol suppress natural testosterone?
Yes. This is one of the best documented methandrostenolone endocrine effects.
A 1975 report examining a male athlete found marked depression of both blood and urinary testosterone during high methandrostenolone exposure. Gonadotropin excretion also fell.[2]
A later report on six bodybuilders who had used methandrostenolone intermittently for a year or longer found testosterone below laboratory reference expectations and reduced luteinizing hormone, despite no subjective sexual disturbance at the time of examination.[3]
A separate controlled experiment in trained men also reported a fall in plasma testosterone while participants received methandienone.[8]
| Level | Possible change | Possible consequence |
|---|---|---|
| Hypothalamus | Reduced reproductive hormone signalling | Less stimulation of the pituitary |
| Pituitary | Reduced luteinizing hormone and follicle stimulating hormone signalling | Less stimulation of the testes |
| Testes | Reduced endogenous testosterone production | Lower natural androgen production |
| Spermatogenesis | Reduced reproductive support | Sperm production can fall |
Read anabolic steroid induced hypogonadism for the wider endocrine record.
Can Dianabol reduce male fertility?
Methandrostenolone can suppress the hormone signals required for normal testicular function. That creates a direct biological route to reduced sperm production.
Earlier anabolic steroid research in mature men also investigated spermatogenesis alongside strength and body composition outcomes, while the broader clinical literature documents impaired sperm concentration during external androgen exposure.
Fertility cannot be inferred from libido, erections or ejaculation. A person can report normal sexual function while testosterone or reproductive hormone signalling is suppressed.
| Measure | What it helps assess | What it cannot prove alone |
|---|---|---|
| Luteinizing hormone | Pituitary stimulation of testosterone production | Current sperm concentration |
| Follicle stimulating hormone | Part of spermatogenic signalling | Complete fertility status |
| Total testosterone | Current circulating testosterone | Conception potential |
| Semen analysis | Sperm concentration, motility and related measures | Exact future recovery from one sample |
Read anabolic steroids and male fertility for the broader fertility evidence.
Can Dianabol affect libido or sexual function?
Yes, although subjective sexual function and blood hormone status do not always move together.
The bodybuilder observations are useful here. Men with suppressed testosterone and luteinizing hormone did not necessarily report subjective sexual impairment at the time they were examined.[3]
Across anabolic steroid exposure, changes in libido, erectile function and sexual wellbeing can occur during use or after endogenous hormone suppression becomes more prominent.
| Observation | What can be true at the same time |
|---|---|
| Normal libido | Natural testosterone can still be suppressed |
| Normal erectile function | Sperm production can still be impaired |
| Low libido | Hormonal, psychological, sleep and cardiovascular factors can all contribute |
| Erectile difficulty | Hormonal suppression is one possible cause among several |
Can Dianabol contribute to gynecomastia?
Gynecomastia is biologically plausible during methandrostenolone exposure because androgen and estrogen signalling can become imbalanced. Methandrostenolone can participate in estrogenic pathways, and external androgen exposure also suppresses endogenous reproductive signalling.
Gynecomastia is well recognized across anabolic steroid exposure, though the direct controlled methandrostenolone evidence is less developed than the compound specific evidence for liver effects, blood pressure and testosterone suppression.
A new breast lump should not automatically be labelled gynecomastia. Persistent unilateral enlargement, nipple discharge, hard tissue or rapidly changing breast findings need medical assessment because other breast disorders can produce similar signs.
What side effects can Dianabol cause in women?
Methandrostenolone has androgenic activity and can cause virilization.
Direct case evidence exists. Two prepubertal girls exposed for months to a cream containing methandrostenolone developed accelerated growth and bone maturation, clitoral enlargement and deepening of the voice. In one girl, the voice change remained years after exposure had ended.[9]
That report involved children and skin exposure rather than adult oral use, so it should not be treated as a dose comparison. It demonstrates that methandrostenolone can produce systemic androgenic effects and that some changes may persist.
| Effect | Possible manifestation | Can complete reversal be assumed? |
|---|---|---|
| Voice deepening | Lower pitch or persistent voice change | No |
| Clitoral enlargement | Androgen sensitive tissue enlarges | No |
| Hirsutism | Facial or body hair growth increases | No |
| Acne | Oil production and inflammatory lesions increase | Often improves, though scarring can remain |
| Menstrual disturbance | Cycle timing or bleeding pattern can change | Recovery varies |
| Scalp hair loss | Androgen sensitive hair loss may accelerate | Regrowth cannot be guaranteed |
Read anabolic steroid effects in women for the wider female health record.
Can Dianabol cause acne or hair loss?
Acne has been recorded in human methandienone athlete research and is a recognized androgenic effect. Increased androgen signalling can increase sebaceous gland activity and worsen inflammatory acne.
Androgens can also accelerate genetically determined scalp hair loss in susceptible people. The extent varies greatly because follicle sensitivity is partly inherited.
| Change | Mechanistic link | Possible long term issue |
|---|---|---|
| Oily skin | Androgen sensitive sebaceous glands | Often improves after exposure ends |
| Acne | Increased sebum and follicular inflammation | Scarring can remain |
| Scalp hair loss | Androgen sensitive hair follicles | Lost hair may not fully return |
| Increased body hair in women | Virilizing androgen signalling | Can persist |
Read steroid acne and hair loss from anabolic steroids.
Can Dianabol affect mood, sleep or behavior?
Anabolic androgenic steroids can affect mood and behavior, but methandrostenolone only evidence is less extensive than the liver, blood pressure and endocrine evidence.
Across the drug family, reported effects include irritability, agitation, mood instability, sleep disturbance, anxiety, depressive symptoms and dependence related behavior. Changes can occur during exposure or after endogenous hormone production has been suppressed.
Attributing one psychiatric symptom to Dianabol can be difficult when stimulants, other steroids, alcohol, recreational drugs, sleep deprivation or existing mental health conditions are also present.
| Factor | Possible contribution |
|---|---|
| External androgen exposure | Can alter neuroendocrine signalling |
| Suppressed natural testosterone | May contribute to low mood, fatigue or libido changes |
| Sleep loss | Can worsen irritability, anxiety and impulse control |
| Stimulants | Can add agitation, insomnia and cardiovascular symptoms |
| Several anabolic steroids | Makes compound attribution difficult |
| Pre existing mental health condition | Can change susceptibility and symptom expression |
Suicidal intent, psychosis, severe agitation or immediate danger needs emergency help. Read anabolic steroids and mental health.
Can Dianabol affect the kidneys?
Direct methandrostenolone specific kidney toxicity evidence is much less developed than its liver evidence. That distinction matters.
Kidney health can still be affected indirectly through blood pressure elevation, fluid balance, cardiovascular strain, dehydration, severe liver illness or use of other substances. Research involving long term anabolic steroid users has also identified renal abnormalities, but exposure often involves several compounds and other risk factors.
| Pathway | Possible renal relevance | Evidence boundary |
|---|---|---|
| Higher blood pressure | Persistent hypertension can damage kidney blood vessels | Direct methandienone evidence exists for systolic pressure elevation |
| Fluid retention | Changes volume and electrolyte handling | Direct human evidence supports fluid related weight gain |
| Dehydration | Can reduce kidney perfusion | Not specific to methandrostenolone |
| Several drugs | Kidney stress can have multiple simultaneous causes | Common limitation of nonmedical exposure research |
| Existing kidney disease | Reduces renal reserve | Requires individual clinical assessment |
Very little urine, rapidly worsening swelling or severe illness needs prompt assessment. Read anabolic steroids and kidney damage.
Why is Dianabol especially concerning during childhood or adolescence?
Methandrostenolone has direct evidence of systemic androgen effects in children.
The two prepubertal girls exposed to a methandrostenolone containing cream developed accelerated growth velocity and advanced bone maturation together with virilization.[9]
Faster growth for a period does not mean a greater final adult height. Androgens can advance skeletal maturation and accelerate growth plate closure, reducing the amount of future height growth available.
| Process | Possible effect | Why it matters |
|---|---|---|
| Growth velocity | May temporarily accelerate | Short term faster growth does not prove improved final height |
| Bone age | Can advance | Skeletal maturation can outpace chronological age |
| Growth plates | May close earlier | Can reduce remaining adult height potential |
| Voice | Can deepen in androgen exposed girls | Change can persist |
| External genital tissue | Can enlarge under androgen stimulation | Some virilizing changes may not fully reverse |
| Normal puberty | External hormones can disturb endocrine signalling | Developing hormone systems differ from adult physiology |
Which Dianabol side effects can develop without obvious symptoms?
Several of the better documented methandrostenolone effects can occur while a person still feels relatively well.
| Possible effect | Can symptoms occur? | Can it remain silent? |
|---|---|---|
| Suppressed testosterone | Fatigue, low libido or sexual symptoms may occur | Yes |
| Reduced gonadotropins | Usually not directly felt | Yes |
| Reduced sperm production | Usually no immediate symptom | Yes |
| Higher blood pressure | Sometimes headache or dizziness | Yes |
| Abnormal liver function tests | Often none initially | Yes |
| Adverse blood lipids | Usually none | Yes |
| Early hepatic adenoma | May later cause pain | Yes |
| Acne | Visible | No |
| Voice deepening | Noticeable | No |
The absence of symptoms is especially weak evidence for liver tests, blood pressure, lipids, reproductive hormones and fertility.
What do controlled human Dianabol studies add to the safety record?
Methandrostenolone is unusual among many historical anabolic steroids because several experiments were performed directly in athletes and other human volunteers.
One double blind crossover trial involved thirteen experienced male weightlifters. Methandienone increased body weight and systolic blood pressure, caused numerous side effects and led three participants to withdraw. Investigators considered much of the added weight to be associated with water retention.[1]
A separate double blind crossover experiment involving trained men found an average body weight increase of about 3.3 kg during methandienone exposure. Plasma testosterone fell. The researchers cautioned that increased intracellular fluid could explain some of the measured body composition change.[8]
The endocrine case research found pronounced suppression of blood and urine testosterone during high methandrostenolone exposure.[2]
| Research population | Relevant finding | What it adds |
|---|---|---|
| Experienced weightlifters | Systolic pressure increased; water associated weight gain and several adverse effects occurred | Controlled evidence in healthy trained adults |
| Athletic men | Body weight increased and plasma testosterone decreased | Shows anabolic appearance and endocrine suppression can occur together |
| Male athlete under endocrine observation | Blood and urinary testosterone fell markedly | Direct evidence of reproductive axis suppression |
| Long term bodybuilders | Low testosterone and luteinizing hormone, abnormal liver tests and metabolic abnormalities were found | Shows silent abnormalities can persist during repeated exposure |
| Patients receiving methandienone medically | Hepatic excretory tests and enzymes changed | Direct evidence of hepatic biological effect |
These trials were small and old, so they cannot calculate modern population risk with precision. Their value is that several widely discussed Dianabol effects were observed directly rather than inferred only from other anabolic steroids.
Can a product sold as Dianabol have a different side effect profile?
Yes. Historic research describes known methandrostenolone. A current unregulated product carrying the Dianabol name may create a separate product identity problem.
If the product contains another steroid, an unexpected concentration or more than one active substance, the observed adverse effects may not match those expected from methandrostenolone.
| Evidence | What it can establish | What it cannot establish |
|---|---|---|
| Dianabol printed on packaging | The claimed product name | The chemical contents |
| Methandrostenolone substance record | Reference chemical identity | Contents of a current tablet |
| Historical Ciba research | Effects of pharmaceutical methandrostenolone studied at the time | Safety of an unrelated present product |
| Seller certificate | A supplied claim | Independent confirmation of the held product |
| Independent chemical analysis | Findings for the tested sample and method | Composition of every later batch |
The product name alone cannot establish that methandrostenolone caused a particular symptom or blood result.
What might a clinician assess after suspected Dianabol side effects?
A useful assessment starts with the claimed product, first and last exposure dates, symptoms, other anabolic steroids, prescription medicines, supplements, alcohol or recreational drug exposure and relevant medical history.
The areas assessed depend on the health concern. Liver symptoms call for a different workup from fertility concerns or chest symptoms.
| Health question | Possible assessment | What it may help show |
|---|---|---|
| Blood pressure | Repeated blood pressure measurement | Current hypertension and trend |
| Liver | Bilirubin, ALT, AST, alkaline phosphatase and other tests based on presentation | Possible hepatocellular or cholestatic abnormalities |
| Severe abdominal symptoms | Physical examination and imaging when indicated | Possible liver, gallbladder, bleeding or other abdominal disease |
| Blood lipids | Lipid profile | High density, low density and triglyceride pattern |
| Kidney health | Kidney function and urine testing when indicated | Possible renal impairment or another illness |
| Hormone suppression | Selected reproductive hormones | Current endocrine pattern |
| Fertility | Semen analysis when appropriate | Current sperm measures |
| Cardiac symptoms | Electrocardiogram, cardiac blood tests or imaging when clinically indicated | Possible cardiovascular disease or injury |
| Blood count | Complete blood count | Provides hematologic context and may identify another illness |
Not every person needs every test. Timing, symptoms, medical history and other exposures determine what is informative.
Read blood tests for anabolic steroid users and talking to a doctor about anabolic steroid use.
What can make Dianabol side effects more concerning?
The same methandrostenolone exposure does not carry an identical health meaning for every person. Existing disease, other medicines and additional drugs can change both probability and consequences.
| Context | Why it matters |
|---|---|
| Existing liver disease | Methandrostenolone belongs to the oral alkylated androgen group associated with cholestatic injury |
| High blood pressure | Controlled research found methandienone could increase systolic pressure |
| Cardiovascular disease | Fluid, blood pressure and lipid changes may carry greater consequences |
| Kidney disease | Fluid and blood pressure changes may be less well tolerated |
| Pregnancy | Androgen exposure can disrupt fetal sexual development |
| Childhood or adolescence | Bone maturation and pubertal development remain active |
| Fertility plans | Gonadotropin and natural testosterone suppression can affect reproductive function |
| Another oral alkylated steroid | Hepatic and lipid burdens may overlap |
| Several anabolic steroids | Endocrine and cardiovascular effects may overlap and attribution becomes harder |
| Unverified product | The actual active ingredient may be unknown |
